Pre-Participation Requirements

The FHSAA utilizes a streamlined process to clear students for participation in athletics. All athletic clearance will be managed through the digital platform, Athletic Clearance (Home Campus).

What is Athletic Clearance (Home Campus)?

Athletic Clearance (Home Campus) is a system designed to simplify and improve the clearance process by

  • Allowing families to complete and submit all necessary forms online.
  • Automatic emails to parents/guardians if there are forms missing.
  • Automatic email reminders to parents/guardians when the EL2 (sports physical) and EL3 (consent forms) expiration dates are approaching.

What You Need to Do

All students that would like to participate in athletics must be entered into this system, even if they have already been cleared and have been participating in conditioning over the summer. To get started, create an account on the platform.

Create Account

  • Please be sure to enter ALL sports that your student will be trying out for or participating in this school year.
  • If you already have submitted paperwork for this year, or the EL2 is still valid from last year, the Health and Wellness Center will enter the physical dates for you after you create an account for your student-athlete.

* Please note that your student-athlete must be fully cleared through the Athletic Clearance system before participating in any practices, tryouts, or games. We recommend completing the clearance process as early as possible to avoid any delays. If students show up at any team practice, open gym, conditioning or tryouts and are not in the system and cleared they will not be able to participate.
