Acknowledgement & Agreement with Oxbridge Academy Foundation, Inc. d/b/a Oxbridge Academy. Release & Indemnity: All reasonable care and supervision will be exercised to provide for the general well-being of my child. However, I understand and assume the risk of injury inherent in camp sports activities. On behalf of myself and my child or legal dependent, I do hereby release, indemnify and hold harmless Oxbridge Academy and its affiliates, and their respective members, officers, directors, employees, agents and volunteers, from and against any and all causes of action, claims, damages, costs, demands and liabilities of any nature relating to death, personal injury or damage to or loss of property which may arise in connection with participation in the Summer Athletic Camps. Registration & Cancellation: I am financially responsible for the Camp(s) for which I am registering. A refund will be granted for cancellations submitted in writing one week prior to each session start. Photo/Video: Photographs and video of the student may be taken, individually and in groups, which may be used in school communications or publications including website, newsletters, advertising and media releases. Behavior: Continued participation in our Summer Athletic Camps is contingent on the student maintaining good behavior.
